I did not receive a printed (and signed) copy of my CAQ. How do I request a new printed copy?

My CAQ renewal was approved, but the hard copy did not arrive in my mailbox.

What is the procedure for requesting a new hard copy of my CAQ?

I assume they issue soft copy and printout of the soft copy should work.
