I am not getting passport request after one year from medical

Hi There

My medical result have been received on dated 14th April'2015 and still i did not get passport request. Please note that already 1 year passed from the date of medical. The web portal shows in process and in the mean time I have collect GCMS notes. Anybody can help me what is the meaning of the below GCMS notes short node:

there must be more details in GCM notes, Please paste those. Look for key word like Security, Medical, eligibility etc.
