you can withdraw via E-cas, fax or postal mail. You will need to provide the following information:

file number, if available
full name (surname, given names)
date of birth (dd/mm/yyyy)
mailing address
e-mail address, if you agree to e-mail communication
telephone and fax number, including area code (example: 101-6532-3236)
