Well as far as I understand from talking to several certified translators (ATIO), the stamp translation would cost $50 per passport (42 pages) plus $5 per non-English stamp. The $50 charge goes toward certifying every single page of a passport (I was said it's ATIO regulation).
The translators I talked to seemed to be flexible in terms of final cost, so you may get some discount if, like me, you have multiple passports or many stamps to be translated.
Also, you may scan and photocopy (some said color-photocopy) all the pages and take them along with passports to the translators. Or, they would do it for you and of course would charge you for that (maybe more than if you'd do it yourself).

Yes of course only the non-english ones. But you need to translate all the stamps that the officer will be looking it. So if your passport is a 10-year and is still valid then you need translation for all non-english one. Otherwise how will the officer know it falls within or outside the period?
$5 a stamp is way too low I think. If you find one with this price please let us know who.
