How to Submit Use of a Representative Form

Hi all;

My wife wants to add me as a "representative" on her file so CIC could release information to me when I call the call centre . I called the call centre and they sent me a link for the form using Web Forms. There is nothing on the site that says how I could submit the form if my file is already in process. Does anyone know how to do it / have done it themselves?

You can just complete it and mail it to your local office if it's been forwarded there from Sydney, or else you can send it to Sydney.
