How to Submit Updated Information Form

Hi All,
Today i recieved Medical Request along with Updated Information Chart document attached and also to pay the Right of Permanent Residence Fees, Can some one guide me how to submit the Updated Information Chart i.e. Via email or courier etc. please guide its urgent

thanks for the replies, the VO asked me to send as email attachment. the questions were simple like name, address, spouse name , and children names (if any)
