How to submit ADR- imm 5669e on IRCC secure portal?

Hello folks,

I received a request to resubmit schedule A. My concern is how to fill all details as there are only 5 rows in the personal history section. If I use an extra sheet, then how to submit it on IRCC secure portal?
Do I need to merge it with imme5669 or submit it separately?

I've been asked to submit it with original signature. Do I just need to type my name with date?

On this separate sheet add the question you are answering. When done, print both files to PDF and merge them (you can google for PDFSam Basic for a open-source tool that can merge pdfs)
