How to submit a request to reissue IMM 5292

My mom is going to apply for Citizenship in a month but she has lost her IMM 5292. How can she apply for a replacement?

You can apply for a replacement or a Verification of status (I'm not sure which one applies) through the same procedure explained on the link above.

About the 2nd and 3rd items in the checklist, do they have to be translated? It seems these two photocopied documents have to be issued outside Canada. Am I right?

Indeed all the documents that are not in French or English must be translated. It's very important.
