The instructions tell you clearly what to do. The instructions did not say you need to prove when you were there....the instructions did not say you need to prove why you were there.

It seems you are referring to the Supplementary Information: Your Travels (IMM 5562) form, if so, these are the clear instructions for answering the questions:



If there is not enough space to provide all the necessary information, use an extra sheet of paper. Print your name and the form's title on the additional sheet.

Question 1

Write your full family name (surname/last name) as it appears on your passport or on the official documents that you will use to obtain your passport.

Write all of your given names (first, second or more) as they appear on your passport or official documents. Do not use initials.

Question 2a

List all trips you have taken outside your country of origin or of residence in the last ten years (or since your 18th birthday if this was less than ten years ago). Include all trips: tourism, business, training, etc.

If you did not travel outside your country during this period, check “did not travel”.

***To "show travel history", all you need to do is write on the form which countries you have traveled to, the dates (month and year) of travel and why you traveled there......you do not need to provide anything else.


It's either you did not understand your consultant's request or your consultant does not know what he/she is doing. You should ask your consultant to clarify the question for you.

thanx alot for such a detailed response. this objection is raised after the medical is done. this is asked second time. my consultant is doing lots of stupidities.

I applied all by my self, this forum helped me a lot to know how things can be done....

I penned down my travel history inn brief i.e departure date, to country, transit if any, period of stay, purpose of stay (as we need to specify only for countries in which we stayed more than 6 months)

in the next line i wrote a detail of why i went there & what i did through out.

Then I typed it in ms word in a good looking table format & took a print.
