How to remove condition on open work permit after medical exam

Hi,

My wife has an open work permit and needs to have a condition removed so she can work in the child care/teaching field. She went for an up front medical exam and now we want to apply to have this condition removed but we are confused what the necessary steps are.

We have found a form IMM 5710 that is titled application to change conditions, but after opening the form it seems to be for applying for a new work permit. Also, at the doctor, we were told we would be asked for the UMI number we received on the E medical form, but IMM 5710 does not request this number, only the UCI number from the work permit.

Does anyone have experience on removing this condition? Do we fill out IMM 5710 and send in the form? Where does the UMI number come into the process?

Any help would be greatly appreciated.

You have to apply for new work permit and pay the 225 fee again. Along with the copy of your med results.
