After looking at the Document Checklist - CIO form, there are some things I don't understand in what to include in the mail to send to CIO. Please help clarify:

1. for all the forms, when it says to write in PRINT, does it mean everything should be in CAPITAL LETTERS, including the duties and other descriptions?

2. under number 2 and 4, is it necessary to include photos and information of my family members, even if they are not applying for immigration?

what if the birth place of one of my family members is unknown? Is it necessary to attach any documents or letter to support or explain why it has been left blank, or do I put UNKNOWN in the field?

3. under number 5, do I need to include IMM5746 if I do not use a representative?

4. under number 8, what are the 2 self-addressed mailing labels? Please explain where to get these, and how to prepare them properly.

5. under number 11, if paying via credit card, should I pay first online before sending the application to CIO, or after? How do I specifically pay via credit card?

6. under number 13, which address is faster in processing my application? For people who have sent to the two different addresses, please give your processing times, so we can determine which is faster.

7. Do I need to include this document checklist form in the entire application to be sent to them?

8. And lastly, how do we compile and arrange all these documents to be sent to CIO? Should they be placed in a folder or any kind of envelope, stapled or not stapled?

1."Type or Print in black ink" this means, you can fill these forms by typing or writing with your own hand-writings. If you choose to fill the forms with your hand-writing, then please use "BLACK PEN".Everything should NOT be in CAPs. use normal writings. Evryting needs to be in Black!
2. Do not send photos to CIO, NS at this time. But, family information is required even if they are not immigrating.
Yes, Unknown is fine. No supporting docs needed at this stage.

4. Send two mailing labels with your own address. Mailing labels are sticker type labels, use to write addresses. CIO will use these labels to send you letters in the future.You can find these in any stationery shop. make sure you have your Postal code in your address too.
5. Please use this form to pay by CC at http://www.cic.gc.ca/english//pdf/kits/forms/IMM5620E.pdf
6. please send your application by courier. (courier addrsss) 7. Yes. make sure you tick checklist appropriately.
8. Place your forms in order of the checklist. staple or (paper clip) the forms individually. (EX: pages in IMM 0008 form as one application and so on..) Place your whole package in bigger envelop (A3 size will do) or courier service have their own envelops.
Please be informed that there will be some changes to FSW program and may be effective from April. consult seniors here.

And lastly, given the changes you mention, would it be better to apply now, or wait until the changes take effect?

The form is the only way to by CC. I paid with bank draft and did not have any issue.
Which would be better, via demand draft or credit card?......

Courier is fater in delivering your application to NS whereas regular post (P.O Box address) is slower due to long holding time at P.O Box and unsafe. Courier does not get hold anywhere ( it does not go through PO.Box) and will deliver to office right away. Both addresses deliver and application is processed at one place.
What happens if I send it to the other address? you application will take almost a month to get to CIO, NS if you are sending from Aisa.

And lastly, given the changes you mention, would it be better to apply now, or wait until the changes take effect? If I were you, I would send my application to CIO, NS during next week, so that it will reach before any changes come into effect. Since your application is qualified under current FSW criteria, I think you should your application right away. But, please consult seniors here.

you have to fill details of family members even if they are not going with u.

Yes, You need to enter your family members even if they are not immigrating with you.
