how to get common law declaration in Quebec?

hello, my and my girlfirend will try to get a common law status in Montreal in the nearest future. If anyone knew where and how can we get it, we'd be veeery grateful

You have to be living with your girlfriend for at least 1 year before you qualify for common law. You have to prove common law relationship in the form of joint accounts, lease, photographs.
