how to get common law declaration in Quebec?

You have to be living with your girlfriend for at least 1 year before you qualify for common law. You have to prove common law relationship in the form of joint accounts, lease, photographs.
I know the requirements, I just want to find out if I have to go to the notary in Montreal or anywhere else to get a confirmation
