hi all,
I have to fill the IMM5645 for a work permit. I have an elder brother who went go to the USA and get married to a US citizen. However, He passed away 3 years ago In the USA in an accident.
My family and my parent stay in Malaysia. After we did a funeral in the USA and Brought my brother's ashes back to Malaysia.
My question is:
What should I fill in IMM5645 in
Marital status: ????? >>> married ???
Present address: ???? >>> should I fill the address in the USA or Malaysia????
Present occupation: ???? should I fill deceased and the date of dead????
Does any give me some advice?
