How to fill CIT 0002 in case of name change

Hi,

In the form CIT0002E, page one, section 6, there is a table for "all names you have used". Since I've had a family name change, I'm going to write my previous and current name in two separate rows. In the third column for the previous name I would write "Name at birth". Now the question is what should I write for the current name in that column?

Hi, thanks for reply. Actually it was a mistake in my national identity document and I had to correct it. The change was made years before I immigrated to Canada. I mean my PR card - as well as my other Canadian documents - are issued with the new last name.

Looking at the selections under "Why was this name used", I would select it as a Alias as I am sure you had used this name for a certain duration of your life.

If you did not use it and it was a mistake in your national identity document, which was corrected right away, then I would leave it out as this was not a mistake on your behalf, but from where it was issued.

actually I cannot use "alias" for a name which is official. To explain more:
1- my last name at birth was AAAAAA which had a mistake in one letter.
2- at age 26 (4 years before immigration) I officially corrected the mistake and obtained new legal documents. Let's say the new name is AAAAB

Now in CIT 00002 form what should I write in the 3rd column of section 6, for the new (correct) name? This is not an Alias or Nickname and it is not my name at birth, but it is my legal name.

So, AAAAAA would be name at birth even if there was a mistake.
I would write AAAAB as name of birth and submit a letter of explanation with your application for submission explaining why you have two name at birth.
