Subject: Correct Information on Sponsorship Application
As I have never received FOSS notes (it's for inland applicants only and I applied outland), I won't be able to deduce anything further from your FOSS notes. That's why I suggested that you can post your question in the Inland 2011 thread that other inland applicants who may have encountered similar issue can share with you what they did so you can follow.
Note: If you're applying from inside canada, why do you need to pay $5? It should be free while you are present in Canada.
