My wife and I just submitted our paperwork this week and mailed it off. We used the new forms. The order for us was exactly as it is listed on the document checklist, however, I did keep the barcode forms at the top. Any extra sheets of paper we filed in regards to a specific question we made sure to write the form number, ‘section’, and question number and then answered the question. We signed all of these forms and dated them too. It doesn’t say this is required, but we figured it was better to sign everything than not. We placed the “extra sheets” after each form it was related to in the packet.

We used 2 separate envelops. The first being our Sponsorship/PR forms, or as the checklist documents - “Part A+B” of the checklist. We used the second envelope for “Proof of Relationship” and we put a label on the forms which indicated what each is for. We didn’t use any binder clips besides for our stack of 20 photos and we used paperclips for my PR Photos. Per the instructions this is allowed. We left everything else unbound and unstapled. My FBI background check was stapled together from the FBI, so I kept that how I received it. I don’t have the checklist with me but this is how our order went (at least what I can remember).

5: Option C is not required, right? Only NOA?

To answer your questions:

#1- I put the Barcode’s at the top of the entire packet/application because it says to keep these on top. When CIC gets them they scan them into the system, and by having the barcodes for the applications + fee’s, they know it’s all there and won’t have to send back the application for any reason. The only reason they send back applications is either the Sponsor Application or PR Application not being filled out completely (at least that’s what people from this forum said). After reading around these forums before sending my packet I was told some people put the Barcodes under the Checklist, and some put them on top of everything- both works, I just figured all barcodes can go at the very top to be safe.

#2- Extra sheets for the questions were signed by either both of us, or just one of us, and dated. The reason I say it depends is because if the form was supposed to be filled out by both parties we both signed it. In my case, I had some forms only signed by me (the applicant), and some “extra sheets” signed by both of us (such as Relationship Questionnaire). For example, my wife signed the form on the Part A of the Relationship Questionnaire which was only to be filled out by the Sponsor. We added an extra form for her employment history and wrote the addresses on it because they want you to write them out in full and the boxes in Adobe didn’t allow us enough space. In the form itself we wrote “Not Enough Space- SEE ATTACHED SHEET.”

So to answer your question, just check to see if the question your answering is supposed to be filled out by the sponsor, applicant, or both – and figure out who needs to sign it from there. I don’t even think this is a requirement; it was just us playing it safe. I wrote our names and DOB on the back of each photo, for both of us, we didn't sign them though.

#3- I just “separated” the Part A documents (applications) from the “Part B” of the document checklist, which includes all of the “Supporting Documents”. It was just to create some organization. You don’t need to do this. I didn’t want a third envelope as I felt like that was unnecessary. #5- Not sure what you mean about Option C? We didn’t include any forms we didn’t need (such as previous relationships, children or other dependents). We also didn’t fill out the forms for Use of a Representative, etc.

Let me know if I can be of any more assistance. I must have read over my forms 100 times before sending them out. Keep in mind you’ll want to probably write-in “Present” on the Relationship Questionnaire form which requests your Sponsor’s employment + address history. We also ended up writing in the Year/Month for the question which asked about visits to see each other. Adobe tries to default it to Year/Month/Day but the forms themselves say YYYY/MM-YYYY/MM, and we weren’t 100% clear on the exact dates we returned from seeing each other (it was 4-5 years ago now). On the “validated” forms I didn’t write anything in empty boxes as they have been validated, but on the Family Information form, and Relationship Questionnaire, you’ll want to put “N/A” in every blank box which doesn’t apply to you.

Great great explanation and thanks a lot for taking this time to give this much info. :)

On 20 pics part, I posted as a separate topic as well. But now as you have submitted the new package, I am sharing my thought with you too. Let me know if I am doing right. So they said 20 pics, I rather made a word file containing 20 pages and 1-2 pics with date taken and context in every document, say around 35-40 pics in total. What do you think?

I sent actual pictures and wrote descriptions on the back of each with our names, but I don't see why that wouldn't work. I know other members mentioned they did that too.

I provided about 14 pages of proof (I know they ask for 10, but we went a little over). I wasn't sure if they meant 10 pages of total social media proof only, or 10 overall - but either way we didn't even include too much social media proof. I showed our facebook relationship/when we met; I printed that piece out and circled the obvious parts and wrote on the back explaining what it was showing.

I also printed 2 pages of facebook chat, which ours did show the facebook logo and chat icon. I think you'll be okay just write on the back explaining what it is. As long as they see the chat and interactions I think you'll be okay.

We included boarding passes when we saw each other, passport stamps, boarding passes from our wedding, a notarized letter from our parents explaining how our relationship is true, how we met, etc. Umm...also included proof showing her as my beneficiary on my 401k/retirement account. We don't share any bills or property together since we don't live together and are recently married. Basically provided proof of our 5 years of dating and stuff best we could with what we had. Lots of Wedding/Christmas/Birthday cards from family and each other as well.

I guess 10 pages consists of Fb, Whatsapp, phone calls, text messages, skype, emails, gifts.
I made it in this way-

Fb - 2 pages of just our message. I took snapshot of the particular message we exchanged and put it in the word doc. By this, I have concluded 2-3 years of communication.
Whatsapp - 3 pages - Took sceenshot on phone for moments, and put it in the word doc.
Phone calls- 2 pages - 1 page from the provider with details and 1 page with screenshots from the phone
Skype- 2 pages
Gifts - 1 page.

What's your thought on this?

I never took any insurance plan, but I got letters from my family and relatives (10 letters). Would that be enough?

Oh no. I recently submitted our application (via the old forms), and while I remember reading that photos should be left loose, I didn't see that for anything else.

Therefore, I stapled pages that were relevant to each other. For example, all pages for a given application form were stapled. Each month of our bank statements were stapled. Photos were left loose however.

Does stapling forms mean they reject the application, or take longer to process?
