How to apply for extension of study permit and spouse work permit

Hii i want to apply for my wife study permit extension and my work permit application together but I don't know how to submit combined application for both ? Thanks

I am not sure but I vaguely remember that in CIC questionnaire you will be asked to submit application for a family member. Let's wait till other members reply.
