How to apply for bridge work permit?

Hi, I have to extend my work permit while waiting for my PR. I am CEC inland. I don't know what to answer for this two question?

Have you been told by an Immigration, Refugees and Citizenship Canada office that you are approved in principle for permanent residence in Canada?

Have you submitted a permanent resident application in Canada?

It also ask me if I have LMIA, when I answer no, it said I need to apply for LMIA to be eligible, any help?
