How to answer " have you ever used any other name?" in Express entry

Hello,
I would like to apply super visa application for my parents.My Mom' Last name is different than all my Educational certificates. Even different from my ID as well. So in that case what should i select? Yes or No? I will upload same name affidavit in the Relationship proof placeholder. Just confused with this question.
