How many police verification needed

Dear Experts,

Below is my over all history of places I live and worked , I am not sure how many police verification letter i need to obtained, appreciate your help.

1981 to 1999 Kanpur India
1999 to 2001 Lakhnow India
2001 to 2005 Banglore India
2005 to 2007 Delhi Inida
2007 to 2008 Germany ( 9 months)
2008 to 2014 Mumbai
2014 to 2015 USA ( 1 year)
2015 to present Banglore India

My best guess is I will need only one police verification for all my stay in India ( regardless of states and cities) , 1 for Germany , 1 for USA. It will be great help if you could confirm on my assumption , please help.

Can you please share details on how you applied for PCC from Germany? How long was the process?
Any helpful links would be useful.
