How many checklist should we fill? + Confusion with the address

Hello again! How many checklist should we fill? Is it one for boths (principal applicant and sponsor) or i should fill one and my husband (sponsor) another one separately?
Plus i must write my address in the forms in Spanish or English? Like, instead of writing "calle principal" (that is the name of the street where I live) should i say "main street?

if one search for main street, will they get to your address ? if no, then dont use the translation. Thats my opinion. But you may wait for other seniors' experience.
