How do you specify single or multiple entry visa on your TRV application?

I noticed that on IMM5257 (TRV application form) there's no mention of single entry or multiple entry visa to choose from? Where do you usually specify which visa youre applying for since i cant find a column for it anywhere on the application form?

Im working in Canada on a work permit and am applying for a TRV from within canada.

You can't specify which. IRCC will decide whether to issue a single or multiple entry.
