How do cic ask for schedule A form

Any one who received schedule A request could you please specify how cic ask for it? Do they clearly mention that they required schedule A?
 
they will send you a message in mycic, to provide sch A form ( IMM 5669 E )and link to the recent form, they will also specify which question they want to be answered particularly, besides filling whole form
 
I received the request for the Schedule A form . They have sent a message and a link for the form . As i have mentioned rejection of study visa, they have asked for the explanation about the particular question where I have put yes .

I have asked seniors in this forum as they said it is the regular process from CIC

Did u not explain in the LOE section provided for additional documents ? Along with your eAPR submission ?
 
I put the explanation letter during submission . But here request is like "Replacement Needed Schedule A" . So I again made an explanation letter with the form and uploaded the pdf

they did not ask me for any sch A form just few documents like birth certificate national id number and any previous arrest and dates.
for the last one I just wrote a letter that I have never been convicted nor arrested for any crime and my pcc is on file.
