Yes you will require further testing.

At a minimum, you should expect this will delay your application. IRCC will request more tests to determine what the cause is of your high blood pressure.

You should work with a doctor to get this issue treated as soon as possible.

I would agree that 200 is extremely high. This generally indicates some underlying problem. And yes, it's possible this could create issues in getting an approval. It depends on what the underlying issue is.
