THNX SO MUCH for replay ok this is my stusation i get mail from the ambassy asking me a new medical test,a new police report and they ask me for that(please document your activities, addresses and list your travels since July 2010)so is that the imm 008????because i dont find any paper to fill out to do that...plus i dont make any trip out the contry from like è years ago so i have to do it pls help me

It should specify which form it is in the letter.
