This is my first post. I am hoping to get some help on completing the medical examination for immigration.

I am an American living (legally) in Madrid, Spain. My wife is Canadian. We are planning to move to Canada in the Summer of 2014. I have completed almost all of the forms required to apply, but I am a bit bewildered by the Medical Examination.

The Canadian Immigration website, lists two medical clinics in Spain that are Panel Physicians. I called the clinic to make an appointment, but they told me that I needed to have the forms already, before I could make an appointment, and that I should contact the Embassy in Paris (as the consulate in Madrid does not do Residency work) to request the forms. I have previously contacted the embassy in Paris, and was told that since my application was a Spousal application that everything would need to go directly through Mississauga.

I wanted to ask for help here, before I contact the embassy in Paris again as they take a very long time to reply.

I just need to know, how to get the forms, and then what the process is after for them after? Do I send the medical forms in with my application or is that done by the clinic?

Hello,

In general medical forms are completed by your doctor, the doctor who takes care of sending them as well, you don`t need to have this forms, not even the application forms.

Only thing you need is an valid passport and 4 small pictures.

I sugest you to call another doctor, call as many as you have on your list.

Another thing, the doctor will give you a copy of your medical form which you have to send it along with your application, the rest of your medicals forms is your doctor who takes care of sending them.

I did my medicals in Paris, the doctor asked me why I chosed Paris, well I said becose I was in my honeymoon there and I decided to do them there since it was very easy.

There are many clinics in Paris where you could do them, I chosed the British Hospital.

Yes, if your doctor doesn't know the procedure, find another one! All you need to do is show up with your pictures and bring the info from CIC to show in case the doctor needs reassurance! You will get a copy of one of the forms and the doctor sends the medical report to either Paris or London. You send your copy of the form with your entire package to your visa office where you're applying. It's very straightforward and the doctor should, in theory, know what s/he is doing! so call around to find a good dr! good luck!

I didnt know you had to take photos as well so that very helpful.

I had the 8 photos taken for the application at studio in Madrid. I took the photo instructions taken from the Western Europe pdf file with me to the studio. It did not specify whether the photos should be cut individually or not. The photographer told me that when they give you such specific photo sizes that it indicates that they would be cut in a machine in Canada. Can you guys tell me whether you sent your photos pre-cut on on the original print-out all together?

the photo place cut ours. just bring in the dimensions and they should be able to do it for you. ours was located in the african appendix, it was pretty straight forward for the photo shop..... I think the doctor had a bunch of forms to fill out and just glued a photo on to each one of the forms. We just gave him the stack of photos and he dealt with it.
