help with gic account !!!!!!

i just received my welcome letter and investment direction but in my investment direction confirmation there is a column for scotia representative and information about her but the blanks for sign and date are empty what should i do with this !!!!

Same question, but I think we need to sign on the bottom one, the other space needs to be left blank I think
