Help! What is Schedule 4

Got new message said need schedule 4, what is schedule 4?
Is it the nomination form I got from cic before? or one form I need fill the information?

They are some simple forms that you need to fill out if you were a PNP nominee. Google "Schedule 4 Provincial nominee" and you should find links to the documents requested. I was confused as well when i got the request..lol.

4a doesn't apply to most of us here.....you can do one of two things:

a) I just filled out what applies and put in N/A for everything else.

b) What most people did was submit Schedule 4 and submitted a letter of explanation for schedule 4a stating that it does not apply to them.

Either method should be fine. I was just following what a friend of mine that applied before me did.
