Help!!Schedule A question about membership or association with organizations

Hi everyone, here's my question. The question 9 in Schedule A about membership or association with organizations. When explaining "City and country", the form says it is the city and country where you were a member. So confused that should I fill the place where I joined the organization or the place where the organization is legally located?

Please help me .Thanks!
