Help! Questions of CIC needs more documents for LCP PR application

I got email from CIC to ask more additional document. The list is 8 and one is asked to fill Generic form for dependents. Who can tell me if I need to fill two this form for each my husband and my daughter?
That mean they are applicant in this form and sign it by me at the last page?
Help me please! It's really comfuse me.

You must complete and sign for all your dependants, including children

This is first document CIC needs in the check list for my defendants.
Is it asked me to fill that form for each defendant and sign it for them? Or I fill this form again?
