Hi All,

I'm an Indian living in the UK from last couple of years. I need to get 'Óne and Same person' affidavit done because my name in education certificate and passport are slightly different.

  1. Can I get the affidavit done in UK? Even though all my documents and passport are from India
  2. If yes, has anyone done it in UK? Can you please let me know where it can be done? What are the documents needed?

i have the same question for Canada. I believe this should be doable.

I am getting it done from Canada. I called up a notary service and they told me they will do it for 89.99 + tax
 
it should be called one and the same person affidavit. Somebody clarify if i am mistaken

Hey, I assume you must have got one for you by now. May I know where did you get it done in Canada? I live in Montreal.

I actually got it from porter law in hamilton, ontario.

It was pretty cheap. I can't remember the exact amount but it was 50 dollars at max.

law firms will know about the format. You just have to provide documents in support of what is going to be included in the affidavit. The person who is going to make the affidavit for you will tell what he/she needs. Just tell him/her that I need one and same person affidavit. Rest they will take care.
