Help! Notarizing an immigration form in UAE!

Hello!

A friend living in UAE and was requested to fill imm 5604 for so she can take her child with her from unaccompanying spouse.

she doesn’t know how to get the form notarized, they keep saying we dont stamp such documents.
Anyone with similar past experience please?

When I needed to notarize a "one and same name" affidavit, the notary office referred me to personal court in Garhoud.
Maybe your friend should try that one.
