Help needed for personal history section

Hi. I am the primary applicant for permanent residence. I was preparing for my IELTS and gathering all my information and documents for some period of months. Now my question is what should I write for those times in the personal history section?

One more question is, I do not remember the address where I travelled (Street number and location) since I was staying at different hotels. So what should I write in the address bar?

Put your permanent home address. Don’t worry about temporary stays. Also yes put ielts months.

If you dont have street number, write your address thats on your passport in a way all the fields have info and full address is entered. Please provide me an example to give you better idea.

Sorry for the confusion but I am not getting able to upload a picture here. below is an example, i visited India for 8 days... now what should i write in the ???? boxes?
