HELP needed - CIC is being weird - CEC applicant

I got a request for an additional document which is the IMM 5604 E - Declaration from a non-accompanying parent.

The problem is that no notary in Ukraine would put their stamp on an English form, hence we did a self-declaration, notarized and translated it. I have attached to my application when I added my dependent.

Now CIC is asling me for the form - IMM 5604 Declaration: Please submit a completed Declaration from NonAccompanying
Parent/Guardian for Minors Immigrating to Canada Form (IMM5604). Note that
a photocopy of a valid and legible identity document of the Non-Accompany Parent/Guardian
must be included. Only official completed forms will be accepted. This must be received at this
office by:


What the hell do I do here? There is no way my ex can complete the form. And I have already submitted the declaration... And now CIC is requesting it again.

Help :(

I certainly would suggest you still notarize it. But if it is nearly impossible, I guess without a notarized copy the form itself should do the work too.

My experience might not apply here. Back then my classmate did not notarize any of his education documents (degree certificate, transcript etc.) and only used the original copy of them to apply study permit and entry visa. And he got his permit and visa just like every other of us who painstakingly notarized every single piece of paper that we submitted.

Or try notarize it in Canada. A quick trip or phone call to a local notary office might clarify a little bit for you.
