Help! Indicate the names of all membership or association with organizations...

In form IMM 5669 it asks:

Indicate the names of all membership or association with organizations you are or were a member of, including:
political organizations,
social organizations,
youth or student organizations,
trade unions, and
professional associations.

The question also wants dates (year, month). I have been a member of a large number of organisations over a period of 40 years, across two countries and a number of different cities. I don't know the dates for most of them, or even which city I was living in for some of them. For some organisations I had periods of membership in one city/country which lapsed and then started again after moving cities and/or countries.

So far, I have spent two months trying to track down information by phoning and writing to various organisations. Most don't have records going back beyond 10 years. This is holding up my application when everything else has been done except this one question.

How do I answer this question?

Do the best you can. Estimate the dates and locations to the best of your memory. List the major organizations, like political parties or unions. Don't concentrate on small stuff like the Star Trek fan association or the Hugh Jackman fan club.

How do I answer this question?

Like keesio stated, estimate the dates and locations the best you can. Then write a letter detailing the situation, so that they know that you are not trying to keep anything from them, but they are too many and too long ago to remember.

Blast! And here I was trying to remember the months and dates of book clubs, breakfast clubs, and all those fan clubs I was a member of when I was eight years old ;D
