Help in filling Finger printing card FD258 for FBI clearance

I am seeking some help to enter a few fields in FD258. Specifically, what to fill in "Reason fingerprinted" and "Employer and Address"? In the application form, I have checked the "Reason for Request" as "Live, work, or travel in a foreign country" but what should be entered in FD258? Also, no idea about what does Employer mean in FD258?

I did my FBI fingerprinting from Local County office in USA. First called to book an appointment for same or next day.

The officer asked me few questions and entered them in system. She verified my ID ( state DL) and asked for my SSN. Then took fingerprints on their machine and printed it on the FD258 then signed/stamped it. I didn't write anything on form myself except my address and signature.
I answered the following to the officer.
Reason for request: Applying for PR in Canada
Employer: Blank. ( The officer said that this background search is not for employment so doesn't matter)
