Help! How to access checklists/forms/guides for the old application package?

I have everything saved and printed off already for the old application package, and know I need to get it there by 1/31. The only problem is that I didn't actually save/download the guide and checklist for the US, which I know needs to be printed and attached to the front of the application. The only guides/checklists I can find are for the new package and have different requirements...

Can anyone point me to where I can download this, or have a copy saved that they could send me??
