Hi,
i need help as i am freaking out and don't know what to do. my husband just received an email from CIC stating this:
• You must provide a current, signed and completed Additional Family Information (IMM5406). This form should be completed for the Principal Applicant (regardless of age) and all dependents aged 18 and over that are listed on the IMM0008 form, whether accompanying or not. For further instructions, refer to the CIC Website at http://www.cic.gc.ca/english/information/applications/index.asp and select the appropriate Family Class category under “Immigrate to Canada Permanently” to access general and region-specific instruction guides and immigration forms.

Here's why I'm confused. I do NOT have any dependents, so I do not understand why I need to fill out this form. It is asking for my mother, father, and sister's information - all of who are NOT my dependants. Do I need to fill out their information even though they are not involved in this process and are not seeking sponsorship? Do I just send them the form with my name and info, and put N/A everywhere else.

Thanks!

You had to fill out that form. That's just your additional family information - you need to list everyone even if they aren't your dependents. So if you don't have any kids just put N/A.

Additional Dependents/Declaration [IMM 0008DEP] is the one you don't need to fill out if you don't have dependents.

Agreed. CIC needs your signature on that form to confirm what dependents you have or don't have. It's a mandatory form that needs to be included regardless of your family composition.

THANK YOU!!!!!!!

Does my husband have to fill it out too if he is the sponsor? I am confused because it says my spouse needs to fill it out. So is it just me as the applicant, or does the applicant AND sponsor both need to fill out separate forms?

Only the principal applicant need to fill out IMM5406

Thank you! This process is so stressful, i nearly had a heart attack when my husband called me. turns out it's not as big of a deal in reality as it is in my own head. I am overnighting a hard copy tomorrow, and emailing it on monday. want to have ALL bases covered!!!! :)

I don't know why you didn't fill out that form because even you don't have any dependents, the first section still asks for your info, then your husband, then your parents. And the final section asks for your brothers/sisters. It didn't say anywhere you're supposed to fill the form assuming your entire family are dependents. Next time if you want to cover your bases, you should fill out all forms, regardless if it applies to you or not, then write n/a. That way, the VO knows immediately that this form does not apply to you, but if you dont send in the form at all, they'll probably assume you forgot it.

At least they're only asking you to send in the one page. I forgot mine (and realised after I'd sent it) and my whole package was returned >:(

Good luck :)

OYE! I probably would have had a bigger mental breakdown than i did if they sent my application back. Fingers crossed this is the only thing they need, and we won't get the package back. Just snail mailed a copy today, and will be emailing it monday AM!. This is so stressful. :-X

note that one reason cic wants all that information is for future reference should you decide to sponsor a relative say three years from your time of landing. If you were to sponsor a family member later they will likely restrict to only those identified in previous application. Just some thoughts.

that is very interesting, thanks for the info! Fortunately, if my family ever decides to immigrate up to Canada, they would most likely apply as skilled workers, since they are in high demand medical fields. Of course, i'm the one who decided to go into business, and don't have that option!

CIC do not make it easy for you, do they! If I remember correctly there is no instruction in the crib for filling in the form and the only mention of it is in the check list and in another obscure crib cincerned as I recollect with adopted children. So they make you think their concern is with dependants and then take you by surprise by requiring you to complete a form concerned with parents and siblings and other non-dependants who may have no interest at all in emigrating to Canada. And, moreover, they do not even ask for addresses and do not explain what they are going to do with all the information they garner! Eemsley is probably right, but who knows! It is about time someone at CIC sat down to redraft the present IMM 5406 which in my experience is the form which causes the most stress to potential immigrants. So join the club of those who fouled it up (me included)!

Are you serious? From what I am seeing she does not make it easy for CIC. The instructions are so clear, they even provide you with a checklist of REQUIRED forms and people still manage to miss them.
She is lucky her whole package was not sent back, so please don't blame CIC for peoples obvious mistakes.
