There are clear instructions on top of the form. "If there is not enough space to provide all the necessary information, attach to this form a separate sheet of paper with further details. Print your name at the top of each additional sheet and indicate the form's title and the number of the question you are answering"

You can use excel, word, pdf, paint, access, every program under the sun, as long as it is on a separate paper as outlined above.

Under point 7 it says give full details... both bachelor and masters.

what should be Name of company, employer, school,facility, as applicable if stay at home can we write NA or Home?

Have a look outside the house, maybe there is a number attached?
