HELP!! Applying for study visa for university

Hello,
I am applying for study visa to enter Canada in university category study permit.

when checking the above link, please replace (dot) with full stop

In the above link checklist, a "Student Questionnaire" is required for submitting the appplication. Please tell me where i can find the student questionnaire form. Please provide link if you can.

Also, do i need to submit original "medical exam" result (it's a piece of paper which doctor gave me when i appeared for upfront medical examination)

I am applying for undergrad degree.

I will be very thankful if anyone can tell me the documents submitted by them for getting visa approval.

please refer the vfs website for that form. You might not find it in pdf but you can copy paste it a word doc and get a print out.
