Help : Applying for PCC from Indian Embassy in SFO

Hi,

I am trying to fill the form required by the Indian Consulate in SFO for the PCC but its so damn vague and I don't want to mess it up.

Please help me if someone has filled in the same form.

On the Personal Particulars Performa form, they ask the current address and the duration of stay. Then in the next line it says the following ::

If you have not been resident at the address given at COLUMN 8(b) continuously for the last one year, please furnish other address(es) with duration(s) resided (Please furnish an additional set of P P Forms for each address with Police station.

So, Since I have stayed in Savannah, GA for more than a year for my Masters, I wrote the address and duration but should I also fill a whole new form for that address? And in that form where it says "Present Address" , should that be of my apartment in Savannah and the two references there?

I am thoroughly confused

Plus can I fill this form on the computer using Adobe PDF or is it mandatory to hand-fill it?
