oh! i thougt everyone here is pinoy..... I'm just new here so i dont know how to comment or reply.

I'm a bit worried of my application sent to CPC-M aug.17 2011 coz one of the application form there ( ADDITIONAL FAMILY INFORMATION ) is not the original form. When i sent my application paper to my hubby through LBC i forgot to signed it so my hubby told me to filled up another one '' additonal family information form'' which i did and instead of mailing it, i just sent it to him by fax which means its only a xerox copy with my signature already. anyone here know if it is valid? anyway its just 1 form only.
