Hello,

I hold a Canadian PR and now, I want to apply for Visitor Visa of my Parents.
My Father's name is listed differently (First Name, Last Name Issue) than on my Passport.
So, while filling form imm5257e for my father, it asks for "Have you used any other Name", should I go with NO ?
And attach a One and Same Person Affidavit to prove this ?

Can anyone please help ?

Does this mean that the name of your dad in your passport is different from the one stated in your his passport/other identity? How different?

BTW submitting just an affidavit may not suffice

yup...its different.
Actually, he happens to use FirstName, MiddleName and LastName (FamilyName).

Name on his own passport is : GivenName = "FirstName + MiddleName" and FamilyName = None
On my passport : GivenName = "FirstName" and FamilyName = "LastName"

Regardless of the sequence/combination in .... is the name the same on your/his passports?

For example, is it John Allen Smith in your documents.... but Ethan Lucas William in his documents?
