Good day,

I received a letter from the immigration-Niagara falls office requesting the following additional forms/documents:

1) IMM5406 (For my husband) - Question: Applicant should be on my husbands name right? (though I'm the principal applicant of the application.

2) Birth certificate of my two dependent children - Question: Should be the original copy or photocopy of the birth certificate ?

3) Question: They specify on the email that should be send ASAP. It should be send through email or mail to Niagara falls office?

Yes, I'm the primary applicant (under H&C). IRCC requested additional form-IMM5406 for my husband. I'm a bit confused with the form:

Applicant - my husband?
Spouse - myself (principal applicant)?

I just want to clarify with regards to this email sent from Niqgara Falls office (See below). I already sent requested documents. I've been waiting for a month now still didnt receive yet my medical request form as stated on the email. I already sent them a follow up as well through email. What will I do?

MEDICAL EXAMINATION (I will send out the forms when I receive the completed applications) At this time, you (and any family member, whether accompanying or not) are required to undergo a medical examination. All family members in Canada or overseas must be examined for medical, criminal and security admissibility requirements, whether they intend to immigrate or not. The medical form has been send to you in a separate email. Please note that Immigration Health Services (RNH) will only accept an immigration medical performed by one of the designated medical practitioners listed on our website at: http://www.cic.gc.ca/pp-md/pp-list.aspx Please contact one of these physicians or clinics and make arrangements for them to complete the medical. This form will be sent to the email address which you had provided to us on your application. The completed form must be sent by you or the physician to Health Management Branch (HMB) in Ottawa. Do not send the forms to our Niagara Falls office.
