My employer is not issuing experience and duties report...what should be the cost of an affidavit to be produced in this context....and what should be the words...should i include my annual salary in it..

All details to be put in there, just like u have in a proper experience letter:
Your name & address, your joining date, designation(s), promotions, awards & appreciations, duties & responsibilites, salary with deductions, your department, reporting officer's details etc, and anything else worthwhile that you can think of.

Explain the reason why you need to write this explanation-letter. Also attach copies of all supporting docs like promotions letters, salary slips etc.

I dont think u need to notarize this, but if you do, the notary will give you the exact cost. Will not be very costly, (around INR500 to INR1000)
