Got the passport back with Visa + Some more questions

Dear All,

For your Information, I got the passport back on end of 27th March with the Visa on it, Alhamd o lillah. planning to travel on 16th May. just wanted to know few more things.

1. Where I can get the Suplementry identification form?
2. Is it wise to fill the PRC form ahead with out mentioning the Phone no?
3. is it necessary to complete the immunization form for my kids by a DMP or any doctor can fill that and sign and stamp those Forms?

One week left for the flight. need some urgent replies.

I thank you all for keep supporting me, in my Immigration process. as soon as I land on 16th of may I will post my landing experience(s).

1. No need of supplementary forms. Just use Excel sheets for details. Properly reference each sheet and give refernece number on declaration form.
2. U do not need to fill any forms. Your immigration document will serve as form for IO. They will do the rest. U do need to have one contact number in Canada that will also be used whle applying for SIN.
3. If u r planning to do the school admission, u need these forms otherwise no need. Any doctor can fill these form (provided by CHC)
