got papers for med exam, my wife and kids refuse to go , I have only 1 week for the dead line, what can I do? can I continue without them?

If you are the main applicant then they your dependents dont have to land with you at the same time.

He is not talking about landing now but medical which is compulsory for him and his immediate family members.
Try to pasuade them and make them realise that they do noy need to land with you as the choice of landing is theirs but that is if you are the principal applicant.
Also examine yourself, if there are things you have done or currently doing that they do not like, make amend and get on with life.

Is your wife divorcing you and she has custody of the kids? Your only chance to continue without them having their medicals is to do your own medical and send that and write a letter of explanation stating that you are now separated from your wife, she has custody of the kids and all of them refuse to go and that you understand that you will not be able to ever sponsor them in the future. If you have any proof of the separation, custody arrangements etc. send that too. If you can get a letter from your wife and or kids stating they refuse to go and know they can not be sponsored in the future and better even if you can get a witness (family member?) to sign that they wrote it, you should send those letters as well.

What an "irony of life"!!!
Here is somebody whose wife and kids are refusing to go for the Medicals, which will eventually land them the Visa on their Passport and on the other hand, are My kids and Wife who ask me everyday when they get back from school and work about any letter from the CIC London.

Reminds me of an African (Nigerian) proverb/adage:

"Eni lori oni fila, Eni ni fila o lori" literally translated, means in English:
"He who has a hat has no head and he who has a head has no hat".

As per the new changes in Buffalo visa office checklist (and of course all canadian visa offices will do the same because it is the regulations of CIC), you will have the choice to have your family members not undergo medical tests but in this case you will never be able to sponsor them under the family class, and if they want to immigrate to canada in the future, they will have to find their own way to do so.

You may choose to have them not undergo immigration medical examinations at this time. Then you confirm that you understand that your family members will not in the future be eligible for sponsorship as members of the Family Class, and they must meet immigration requirements in their own right if they wish to join you in Canada.(in this case, You must complete and return the form entitled
"Clarification of Regulation 117(9)(d) - Exclusion from Membership in the Family Class" with your application (attached at the end of Appendix A)"
