Visitors visa does not require medical as the stay will be under 6 months. Only biometrics.
Usually now they ask for both biometrics and medical at the same time for sponsorship. Just wait a bit and you should receive the request.

I got yesterday and today as well...It seems ghost update is not something what people assume that their file is opened or reviewed by case officer. In my opinion when they send pprs, pre arrivals and correspondences to other people then your application is also reflected with updated date as well. Correct me if am wrong here

Yup, they mean zilch. I have had a pile of these "updates" in the last 4 months, but as per my MP and the IRCC call center nothing has begun on my application.

Thank you. I was surprised that I received PPR within a day from pre-arrival. I received it at 9.24 am today. You'll get yours soon, just be patient

I forgot to mention. You can do upfront medical if you like. You just go to an approved medical examination center and tell them you want to do upfront medical for your application. Provide them with AOR and details. Once you receive the results you can update IRCC using webform. A lot of people do this for student visa etc.

Do you know the cons of providing an upfront medical?
If your file is not finalizing within 12 months, you must re-do it again.
Since Dakar is one of the slowest VO, I would wait a little bit.
