· One photograph of your spouse. The photograph must comply with the
Photo Specifications.
· Notarized copy of your spouse’s original hospital birth certificate. For persons born in the People’s Republic of China: You must submit a notarized copy of the original hospital birth certificate issued by the PRC Ministry of Health.
· Copy of your spouse’s
Hukou (household register), if applicable.
· Notarized copy of your marriage certificate
· Photocopies of the biodata pages of your spouse’s passport(s) or travel document(s) with at least six (6) months before expiration.

Police certificates from every country where your spouse has lived for a period of six (6) months or more since turning eighteen (18) years of age.
