Current country of residence ...the from and to dates ...i have been in Canada since 2007 with a study permit till JUNE 2012 2012 and then transferred to PGWP IN JUNE 2012...SO THE FROM AND TO DATES SHOULD MATCH THE WORK PERMIT DATES OR MY INITIAL START DATE IN CANADA

I have the same question, just to clarify: I lived in Canada on a study permit from 2008 to 2012, and now I am here on a PGWP. To answer Q10 I should put that i am here as a worker, and then put the date that i originally came to Canada on a study permit?

The day you entered canada when you had a student permit is the beginning if being a resident. Then start date if work is when you got the work permit

I also happen to be filling this application out right now and I have a few questions:

Question 4: Immigration office requested for processing this application: CPP-Ottawa or CPP-O or what?
Question 4 in Education and occupation details: Intended occupation: ??? Are they asking what I'd like to be after I get my PR? Can I just put my current position as intended occupation?
National identity document: what is it? I am not sure my country has one. How do I find out?

Please help whoever can!!! Thank you in advance!! :D

You can put whatever you want in that. Current occupation or what you want to be after PR.
Honestly I have answered this form quite a bunch of times for different applications, I've also changed my mind a bunch of times of what I wanna do. So it doesn't make a difference if you ask me.

If you are not sure or don't have one just put N/A
some country like the US if I'm Not mistaken have a centralized ID system. My country doesn't have one so I put N/A
